- Go to: http://www.chattanoogastate.edu/
- Click on Admissions in the grey toolbar.
- Click Forms from the website or the link here.
Some computers have the software that will allow you to fill the form in on the computer and email it through the Share button that is located in the top right tool menu on the form's frame.
If your computer does not, just print the form and mail it to the address on the form. You may also hand deliver it if you are on campus.
Having the correct address on your records is important in receiving financial aid checks and other important mail.
